For young people under the age of 18 years old serving in church teams and activities.
We are delighted to have your young person joining our activities at Christ Church. We take Safeguarding of your child and protection of your personal data seriously; therefore, we require you to fill in this form. The information that you give us will be stored on record by Christ Church Office and made available to the relevant team leaders. Photos and videos may be taken of young people while they serve in Christ Church teams and activities. These may be used in church displays, news updates, publicity, our website and social media, and will be stored securely. We need your permission before taking or using any photos or videos that include your child.
