Cost: £6 per head (for 2 games) - bring money on the day

By completing this form you give permission for your young person to take part in the Breakout Laser Tag trip. They will be supervised by Breakout leaders throughout, but please be aware that laser tag is an active game played in a darkened arena and involves running, hiding, and using laser equipment.

If you require a lift to the venue please fill out the information below. Spaces are limited so if you can arrange your own way there, please do so. Drop off time at church is 5:50PM and pick up time at the church is 8:30PM.
How many lift spaces are required
